Camden Integrated Digital Record (CIDR) – it’s your personal jigsaw

After being discharged from hospital you are sat with your GP who has not yet received your hospital discharge letter.

Your GP needs to adjust your medication but needs to see information from your hospital visit first. Your GP asks your permission to view your Camden Integrated Digital Record in order to be able view this information quickly in one place.

Whilst there your GP can also ensure that you are receiving all the care you need at home.

NHS Camden CCG are linking health and social care information in Camden to improve the care Camden residents receive.

The Summary Care Record – your emergency care summary

You are unconscious in A&E after being in a car accident. The clinicians need to know what medication you are on and if you are allergic to anything, so that they can treat you.

By looking at your Summary Care Record they can see that you are allergic to penicillin and that you are on warfarin. They now know not to give you penicillin, or anything that will react with your warfarin.

The Summary Care Record contains information about your allergies, medications and reactions you have to medications, so that in an emergency or when your GP practice is closed this information is available so that you can be cared for.

Care.data – better information means better care

Some medications can have negative side effects, such as thalidomide. In order to be able to identify these side effects early, large amounts of data need to be collected.

By collecting large amounts of data, research can be done which identifies these links, improving the quality and safety of healthcare given by the NHS.

The Health and Social Care Information Centre (HSCIC), which is part of NHS England, is collecting data to enable quality improvement in healthcare.
