We are continuing our attempts at improving access to our appointments.

In order to improve your access to a GP, we will now be offerring routine appointments from 7.00am on some mornings and from 9am to 12am on some Saturday mornings.

Help us to keep your clinical record up to date. Please click on the "clinical record" link at the bottom of this page to update us.

Our "self check area" is now open. Appointments are no longer needed for routine BP checks. We now have a dedicated area in the waiting room eqipped with state of the art electronic equipment with which to measure your own BP. Receptionists will be on hand to assist you. Height and weight can also be checked.

The Minor Ailment Scheme can enable you to get prompt treatment for a range of minor health problems. For further details see the link under Practice Information.

Our Practice Catchment Area

If you live within reasonable walking distance from the practice you may be eligible to register with us.  Please enquire with the reception desk as to whether you currently live within our catchment area.

If you live within the practice area and wish to register with the practice you can do so simply by bringing proof of address to our reception desk during opening hours.  You will also be asked to provide your NHS number as Camden PCT now require this information before allowing anyone to register with us.  You will be asked to complete a registration form and a patient health questionaire at the time of registration the receptionist will assist you in taking your blood pressure. In order to save time you can print off the registration form below which can be completed and brought to the practice at the time of registration.  You will still be required to attend in person in order to complete our health questionnaire and show us proof of address before you are fully registered.  Please note that a separate form will be required for each family member who registers with us.

How do I find my NHS Number?

It is important to ask all new patients (whether registering permanently or temporarily) to provide identification upon registering.

One item of photo ID is required, along with one document containing the patient’s address:

  • ·      birth certificate
  • ·      marriage certificate
  • ·      medical card
  • ·      driving licence
  • ·      passport
  • ·      local authority rent card
  • ·      paid utility bills
  • ·      bank/building society cards/statements
  • ·      National Insurance number card
  • ·      payslip
  • ·      letter from Benefits Agency/benefit book/signing on card
  • ·      papers from the home office
  • ·      P45

The following documents or similar types of documents would not be accepted or considered proof:

  • ·      library card
  • ·      video rental card
  • ·      health club card
  • ·      private rent book
